Post Acute Care Opening in Middletown, DE Wilmington 24m We are seeking a full-time physician to provide patient care in an Inpatient Rehab Facility located in the Middletown, Delaware, area. * Rounding at an Inpatient Rehab Facility * Monday - Friday * Day call weekend rotation * Flexible rounding schedule * Estimated compensation range is $223,000 to $273,426 annually with no cap on productivity income potential. * Uncapped earning potential * Excellent benefit package * Paid professional liability insurance with tail coverage Duties * Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care * Collaborate with clinicians, facility leaders and others regarding care and services * Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery * Provide oversight, guidance, and mentorship to our nurse practitioners * Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care * Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues Qualifications * 1+ years experience in internal medicine or related specialty * Post-acute care experience a plus, but not required; candidates with internal medicine, family medicine/primary care, emergency medicine or related experience welcome to apply * Active and unrestricted Delaware license * Active and unrestricted Delaware DEA Middletown, DE Located 24 miles south of Wilmington, Middletown is a small town in New Castle County, Delaware, with a population of about 20,000.
