Overview:
We are seeking a board-certified/board-eligible Hospice & Palliative Care Physician who can also provide inpatient Hospitalist coverage at a well-regarded hospital system in New Haven, Connecticut. This full-time, permanent position offers a rewarding blend of continuity-focused palliative care and dynamic hospitalist practice within a supportive, team-based environment.
Job Setting:
Work within a multidisciplinary palliative care team committed to providing comfort-focused, holistic care for patients with advanced illness. Collaborate closely with hospitalists, specialists, nurses, and case managers to optimize patient outcomes. The role includes scheduled hospitalist coverage and participation in family-centered, goals-of-care discussions.

Key Responsibilities:
• Provide direct palliative and hospice care in inpatient and consultative settings
• Manage complex symptoms and coordinate care plans aligned with patients' goals and values
• Serve as a hospitalist during scheduled inpatient coverage shifts (rotational)
• Collaborate with nursing, case management, and specialty teams for integrated care delivery
• Participate in interdisciplinary team meetings and quality initiatives
• Provide guidance and support to patients’ families and caregivers
• Educate medical staff on best practices in palliative and end-of-life care
Requirements:
• MD degree from an accredited institution
• Board Certified/Board Eligible in Internal Medicine or Family Medicine
• Fellowship trained and/or Board Certified in Hospice and Palliative Medicine
• Active or eligible Connecticut medical license
• Experience in both palliative care and inpatient hospital medicine strongly preferred
• Exceptional communication, empathy, and collaborative care skills
• Commitment to improving quality of life for patients with serious illness
