Job Summary:
Community Medical Group is seeking a Board-Certified Family Medicine Physician to provide comprehensive, patient-centered care in an outpatient primary care setting.
Job Responsibilities
Includes some Chronic Pain Management
Schedule = 18 PPD, minimum see 18 PPD
A mix of peds/adults (Will consider IM only)
We will also accept DO
BE will be considered if truly BE (5 years or less since residency)
Active DEA needed before the start
Monday-Friday 8-5
Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely)
Qualifications
Diagnose, treat, and manage acute and chronic conditions
Coordinate patient care with specialists and other healthcare providers
Maintain accurate electronic medical records (EMR)
Educate patients on preventive care, lifestyle modifications, and chronic disease management
Participate in a collaborative, team-based care model
