JOB SUMMARY: The hospitalist is responsible for provision of direct in-patient care to adult non-surgical patients of this Health Center. Provides in-patient and out-patient consultation in internal medicine and participates in an out-patient clinic designed to provide consultations and problem oriented care to non service unit patients. Provide medical services and contribute to the design and delivery of high quality, cost effective, values- based patient care for our patients.

Job Responsibilities

ESSENTIAL FUNCTIONS:
1. Direct care to adult non-surgical hospitalized patients
2. Consultation for hospitalized patients
3. Coordinate internal medicine subspecialty care
4. Communicate accurate and pertinent information with patient care providers and other members of the care delivery team to facilitate effective and efficient patient care.
5. Participate in and provide "on-call" services after hours in accordance with schedules to be determined by the facility so that coverage during weeknight, weekends and holidays is maintained so that continuous, uninterrupted adequate patient care is provided.
6. Prepare medical record entries as required by facility policies, medical staff by-laws and regulatory authorities.
7. Participate in medical staff/hospital committees as appropriate.
8. Facilitate patient education and maintain positive patient relations.
9. Consultation for non service unit patients by telephone or site clinic
10. Participate in medical staff quality improvement and review activities

ADDITIONAL RESPONSIBILITIES: May serve as a collaborative practice provider for Advanced Registered Nurse Practitioner. The physician shall provide general supervision for routine health care and management of common health problems, and provide consultation and/or accept referrals for complex health problems. The physician shall be available by telephone and pager services in emergencies and after hours, and when not physically available on the premises where care is given.

QUALITY ASSUARANCE PLAN: Collaborating physician on a monthly basis will review ARNP medical records. Review of medical records will consist of 10% of random sampling of all records of patients within the designated calendar interval. Review of 100% of records for patients with adverse outcomes will be performed.
