Responsibilities:
  • Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
  • Drive high-quality, safe, and cost-effective patient care
  • Collaborate with interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Provide oversight, guidance, and mentorship to clinicians within facilities
  • Schedule: Monday through Friday rounding on 3 different post-acute facilities (not all in same day) from West Palm Beach to Delray Beach; ideal candidate would live in between both locations
Qualifications:
  • Background in providing primary care in internal medicine (IM), Family Medicine (FM), emergency medicine (EM), urgent care, physiatry, PM&R, or geriatrics
  • Ability to work flexible schedules
