Post Acute Care Opening in Las Vegas, NV
- We are seeking a physician who is board certified or board eligible who has a passion to serve patients in all levels of post-acute care including LTAC, post-acute rehab, inpatient rehab and skilled nursing.
- This opening will cover multiple sites in Las Vegas.
Position Highlights:
This is a flexible schedule M-F, with no start/stop times, uncapped bi-weekly bonus based on bi-weekly encounters and RVU mix.

Responsibilities:
Work alongside pharmacists on medication management
Complete Medicare required visits
Complete acute, follow up, and discharge visits as necessary
Implement an individualized plan of care for patients' goals
In collaboration with the IDT determine transitional care
Make recommendations on DME, home health services, or outpatient services after short stay
