PCMH Physician provides direct care to, and assumes responsibility for, all patients in their panel. They also may supervise trainees providing direct care to patients within the PCMH, providing high quality teaching and clinical care. In addition, they oversee clinical panel management provided by the entire PCMH team and collaborating with and supporting the Nurse Care Manager in the care of high-risk patients. The physician works with all team members to achieve and maintain desired patient outcomes in a cost-effective manner. The physician plays a central role in the development, implementation, and evaluation of shared care plans for individual patients and groups of patients in collaboration with other PCMH team members.

Position Requirements

  • Candidates must have an MD, DO or equivalent degree.
  • Candidates must be board-certified or board eligible in Family Medicine, Internal Medicine, or Internal Medicine/Pediatrics
  • Candidates must have or be eligible for a California medical license or equivalent certification/permit as determined by the Medical Board of California.
  • Provides direct patient care.
  • Provides supervisory patient care and support for physician trainees.
  • Clearly documents all patient encounters in the electronic record on the day of the visit.
  • Reviews and acts upon diagnostic test results as appropriate; ensures patients are aware of test results; documents acknowledgement of results and actions taken in patients’ ORCHID record.
  • Responds within 3 business days to inbox messages, proposed orders, and notifications from other DHS staff; some orders may require a prompter response (e.g., medication renewals).
  • Leads multidisciplinary family conferences for high-risk, complex cases as needed.
  • Closes care gaps using ORCHID’s Health Maintenance tab or Registries widget.
  • Communicates patient care priorities, especially follow-up plans that involve non providers (e.g., high-risk patients that need Care Manager follow-up) to medical home team members.
