As a post-acute care medical director and primary care physician you will be a part of an integrated care team providing primary care services in post-acute care facilities including skilled nursing and assisted living. Our patient population includes long-term care, short-term rehab, memory care, and clinically complex patients and residents.

Our physicians are responsible for driving high quality, safe and cost effective care of patients in our local post-acute care communities. You will work on an interdisciplinary team rounding on all patients as medically necessary as required by CMS.

This includes but is not limited to H&P, admission, progress, transfer, discharge, and acute/follow up. You will also work with the facility to transfer patients to the level and site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and/or memory care. Our clinicians work closely with hospice, palliative care, and home care agencies to ensure a safe transition home. Our clinicians are also responsible for timely documentation and billing through our EMR (Gehrimed) that meets Medicare guidelines.

The facilities are beautiful -top of the line! They offer a mix of short and long-term patients, with a higher volume of short-term rehabilitation.

TeamHealth will help you develop a collaborative team approach between the facilities clinical team, supervising physician in the market, and additional service providers in wound care, behavioral health, or infectious disease specialist both locally and nationally. If you have a background in providing primary care in internal medicine, family medicine, emergency medicine, urgent care, physiatry, PM&R or geriatrics this could be a great fit for you.

The typical schedule is Monday through Friday with flexible daily hours. We have an overnight call service provided 7pm to 7am by our advanced practice clinician call team. Clinicians rotate backup call to the call team for occasional escalation needs in the local market and weekend day call.

Our compensation is a productivity based model with a base salary for full time providers that offers autonomy and control to maximize your income potential.
