Our physicians practice medicine in a collaborative manner amongst many medical specialties and sub specialties embracing technology and teamwork to provide patient centered care. • Provides clinical leadership and expertise within the palliative care services team to improve patient care for patients.
• Builds effective relationships with referring physicians to ensure excellent team follow-up regarding patient consults.
• Proactively identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resources.
• Ensure compliance with legal, regulatory, and clinical policies and procedures
• Provide consultation services in palliative care
• Facilitate clarification of patient and family goals of care
• Diagnosis, treatment, and management of acute and chronic health
