Board Certification in Palliative Medicine and/or Geriatrics preferred
DEA certificate
Illinois State Medical License and Controlled Substances License in good standing
Inpatient only, no clinic
No call, no weekends
10-12 patients per day
Start Date: ASAP – 3 month minimum with potential to extend
Hours: M-F 9a-5p, no call, no weekends
Scope of work:
- . Patients May include sickle cell, Hem/Onc, Cardiac, Med/Surg ICU, Renal, Dementia, Neurosurgery, general medical. All palliative care patients.
- · Provide inpatient consultation services in palliative care.
- · Facilitate clarification of patient and family goals of care.
- · Consult with the attending physician and the Interdisciplinary Team to establish a written plan of care at intervals specified in the plan.
- · Facilitate access to appropriate supportive care services.
- · Full and appropriate documentation of patient care services to support professional billing for these services.
- · Participation in on-call coverage with the interdisciplinary team.
- · Assist in the development of standards of care.
