Responsibilities
- Provide clinical oversight for hospice and palliative services, including medical management of patients with serious, chronic, and life-limiting illnesses.
- Certify and re-certify patient eligibility for hospice and palliative care programs in accordance with regulatory standards.
- Collaborate with physicians, advanced practice providers, nurses, social workers, spiritual care providers, and other team members to deliver comprehensive, patient-centered care.
- Serve as a consultative resource to attending physicians regarding complex symptom management, goals-of-care discussions, and end-of-life planning.
- Develop, review, and update medical standards, policies, and procedures for hospice and palliative services.
- Participate in quality assessment, performance improvement initiatives, and internal committees.
- Educate clinical staff through in-service training and support community outreach and professional education efforts.
- Support the development of a home-based palliative care program, including direct visits and care coordination.
- Act as a liaison with hospitals, community physicians, and healthcare organizations to promote awareness and access.
- Provide coverage and support to staff for medical services during absences and after hours.
- Maintain ongoing communication with patients' families, caregivers, and interdisciplinary teams to ensure quality, coordinated care.
Requirements
- Minimum of one year of physician experience, with specialized training in palliative medicine.
- Completion of a Palliative Medicine Fellowship Program preferred.
- Board certification in Internal Medicine or Family Practice.
- Unrestricted Medical License in New York State and valid DEA license.
- BE/BC in Palliative Care.
- Full clinical skill set with experience working within interdisciplinary teams delivering hospice and palliative care.
- Strong communication, leadership, and collaborative skills.
- Additional experience in pain and symptom management, community networking, and program development is advantageous.
- Willingness to work in-person across various community settings and participate in educational initiatives.
