Palliative Physician Opening in CT Located in Hartford Boston 2hrs; New York City 2hrs Full-time; permanent Seeking a BE/BC Hospice and Palliative Care Physician to join an established hospice program within a leading integrated health system in Hartford, CT. Practice alongside nationally respected colleagues in a progressive, physician-led environment with extensive system-wide support. Practice Highlights
- Home-based and inpatient hospice care
- Interdisciplinary team model with individualized care planning
- Home visits both in-person and virtual
- Pain and symptom management consultation
- Medicare Hospice Benefit certification and recertification
- Staff and volunteer training involvement
- Schedule: 48 on-call weeks per year, Monday 7am–Friday 7pm; 4 weekends per year, Friday 7pm–Monday 7am
- One in-person Interdisciplinary Team Meeting per week Qualifications
- BE/BC in Internal Medicine, Family Medicine or Hospice and Palliative Medicine
- Hospice or Palliative Care experience preferred
- Eligible for Connecticut medical licensure Compensation & Benefits
