Lead Physician - Palliative Care - 60% Remote / 40% Home Visits

Our client is seeking a Lead Physician to join their team and offer Palliative Care around Atlanta.

In the role, you will work from home 60% of the time and offer telehealth services, while supporting field-based Nurse Practitioners. 40% of the time you will see patients in their homes.

Schedule:

  • Full Time: Monday – Friday, flexible start/end time
  • 60% remote/work from home, 40% home visits (3-6 patients/day)
  • No weekends or on-call
Compensation: Base salary starts at $250k for less experienced Physicians and increases based on your years of experience, plus a 10% performance bonus

Lead Physician Responsibilities:
  • Perform comprehensive physical evaluations for an Adult Geriatric population
  • Provide in-home and virtual patient care, evaluating and treating symptoms impacting quality of life (pain, anxiety, insomnia, depression, etc.)
  • Conduct goals of care discussions, advance care planning, and family meetings; document patient wishes in advance directives or POLST forms
  • Work collaboratively with an interdisciplinary team to ensure continuity of care
  • Refer patients to appropriate specialists or hospice care when indicated
  • Track and report clinical and financial outcomes using established KPIs
  • Prescribe all necessary medications and treatments
Qualifications:
  • Doctorate Degree in Medicine (MD/DO)
  • Board Certified in Hospice and Palliative Medicine
  • Current, unrestricted medical license; willingness to obtain multi-state licensure, if required
  • Experience with Medicare Advantage, managed Medicaid and commercial populations
  • Skilled in goals of care discussions, advance care planning, and managing complex medication panels
  • Demonstrated ability to integrate behavioral and acute care and address social determinants of health
  • Reliable transportation for occasional patient visits
