Join a patient-centered hospice and palliative care team dedicated to providing the highest quality end-of-life care with dignity, comfort, compassion, and respect for both patients and their families.
* Collaborative multidisciplinary care model, working alongside physicians, advanced practice providers, nurses, social workers, counselors, nursing aides, chaplains, and trained volunteers
* 100% outpatient role, providing home visits and face-to-face care in skilled nursing and assisted living facilities
* Service area includes Moore and Hoke counties
* Mileage reimbursement provided for travel
* Part-time opportunity (20–30 hours per week) offering excellent work-life balance
* Support from an 11-bed inpatient hospice house located on a peaceful campus near a regional medical center

Qualifications
* Board Certified in Internal Medicine or Family Medicine with significant experience caring for adults with acute and chronic illness
* Board Certification or Eligibility in Hospice & Palliative Medicine preferred but not required
* Preference for candidates with five or more years of hospice or palliative care experience
* Eligible for or holding an unrestricted North Carolina medical license
* Ability to travel between facilities and for home visits
* Strong passion for end-of-life care and comfort working as an integrated member of an interdisciplinary care team
