  • Other details: The facility which is stroke-certified is looking for a provider to cover on-site clinical hours and beeper call if needed. Seeking one full-time locum hospice provider for a Monday–Friday schedule from 8am to 5pm with a mix of onsite and telehealth responsibilities, noting that candidates should be willing to work onsite if required in the future. Guaranteed coverage is eight hours per day with overtime applying after nine hours. Call duties include 4–6 weeknights per month and approximately one weekend per month under a shared call structure, with call volume ranging from minimal symptom management to multiple complex admissions, crisis visits, and GIP evaluations, sometimes with NP support. The provider will primarily support a census of approximately 62 patients while serving as backup for an additional service line, and will manage IDT participation, consultations, medical consults, telehealth visits, and CTIs. Candidates must hold an active OR license and DEA, be Board-Certified or truly Board-Eligible within five years of residency, be trained in Family Medicine or Internal Medicine with strong hospice experience, and maintain current BLS certification.

Job Responsibilities

* Provide full-time hospice care via a combination of onsite and telehealth services.
* Manage patient admissions, symptom control, and complex hospice cases.
* Participate in interdisciplinary team (IDT) meetings, consultations, and medical consults.
* Conduct telehealth visits and complete care transition interventions (CTIs).
* Respond to patient and staff phone calls during scheduled call coverage.
* Collaborate with nurse practitioners and other hospice team members as needed.
* Maintain accurate and timely documentation of all patient care in the EMR.

Qualifications

* MD or DO degree from an accredited medical school.
* Board-Certified or Board-Eligible within five years of residency.
* Board certification in Family Medicine or Internal Medicine, with strong hospice experience if FM or IM.
* Active Oregon medical license and DEA registration.
* Current BLS certification.
* Experience managing complex hospice patients.

Working Hours

* Monday to Friday, 8am to 5pm, with flexibility for lunch during the day.
* Guaranteed eight hours per day; overtime applies after nine hours.
* Call coverage 4–6 weeknights per month.
* Approximately one weekend per month of call.
* Shared call structure between two service lines.
