- Other details: The facility which is stroke-certified is looking for a provider to cover on-site clinical hours and beeper call if needed. This locum hospice opportunity is seeking one full-time provider to deliver a mix of onsite and telehealth care, with a preference for candidates willing to work onsite, as onsite coverage may become a requirement later in January; however, telehealth-only candidates will also be considered. The provider will work Monday through Friday from 8:00 a.m. to 5:00 p.m., with lunch taken as able, and will participate in shared call coverage 4–6 weeknights per month and approximately one weekend per month, with call volume varying from minimal admissions and symptom calls to multiple after-hours admissions, crisis visits, and GIP evaluations, sometimes with NP overlap. Weekend coverage may be busy depending on patient acuity and admission volume, with NPs often managing symptom calls while the physician focuses on admissions when overlapping. Call responsibilities are shared between two service areas, with a current primary census of approximately 62 patients, and duties include IDT participation, consultations, medical consults, telehealth visits, and CTIs. Requirements include an active Oregon medical license and DEA, board certification or true board eligibility within five years of residency, a background in Hospice preferred or Family Medicine/Internal Medicine with substantial hospice experience, and current BLS certification.

Job Responsibilities
* Perform patient admissions, symptom management, crisis evaluations, and GIP assessments
* Participate in IDT meetings, consultations, and medical consults
* Conduct telehealth visits and complete CTIs
* Collaborate with NPs and interdisciplinary team members to coordinate patient care
* Respond to after-hours calls, including weeknights and weekends, as part of a shared call rotation
Qualifications
* Hospice experience strongly preferred; Family Medicine or Internal Medicine candidates must have substantial hospice experience
* Active Oregon medical license
* Active Oregon DEA registration
* Current BLS certification
Working Hours
* Shared call coverage 4–6 weeknights per month
* Approximately one weekend of call per month
* Mix of onsite and telehealth coverage, with onsite work potentially required later
