Seeking a board certified Palliative Care Physician for Medical Director role in Hartford, CT.

  • Lead a team of hospice physicians and per diem APRNs caring for patients across Connecticut
  • Participate in the decision-making process in relation to admission to the hospice (homecare) or inpatient setting
  • Participating as a member of the Interdisciplinary Team in the development of individualized care plans
  • Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit
  • Consult with attending physicians regarding pain and symptom management
  • Coordinate efforts with the attending physician to provide continuity of care
  • Develop and monitor the medical component of the hospice program
  • Participate in the development, coordination and implementation of hospice services and policies
  • Participating in the development of staff and volunteer training
  • Conducts Face to Face visits for hospice admissions and recertification when appropriate as required by CMS.
  • Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community
  • Perform related administrative functions as required and/or assigned
  • Participating on the Professional Advisory Committee
  • Establish health policies for employees when requested
  • Participate in the legislative process as it relates to hospice
  • Attend / participate in Hospice QAPI Team meetings and initiatives as requested
  • Full or part time opportunity available
