Seeking a board certified Palliative Care Physician for Medical Director role in Hartford, CT.
- Lead a team of hospice physicians and per diem APRNs caring for patients across Connecticut
- Participate in the decision-making process in relation to admission to the hospice (homecare) or inpatient setting
- Participating as a member of the Interdisciplinary Team in the development of individualized care plans
- Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit
- Consult with attending physicians regarding pain and symptom management
- Coordinate efforts with the attending physician to provide continuity of care
- Develop and monitor the medical component of the hospice program
- Participate in the development, coordination and implementation of hospice services and policies
- Participating in the development of staff and volunteer training
- Conducts Face to Face visits for hospice admissions and recertification when appropriate as required by CMS.
- Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community
- Perform related administrative functions as required and/or assigned
- Participating on the Professional Advisory Committee
- Establish health policies for employees when requested
- Participate in the legislative process as it relates to hospice
- Attend / participate in Hospice QAPI Team meetings and initiatives as requested
