Coverage Needed:
- Start Date: December 2025
- End Date: Ongoing
- Schedule: 8-hour day, Monday - Friday
- Full-time coverage, 4 weeks a month or less
- Call: 3 weeks telehealth on-call, 1 week in-person
Clinical Details:
- Long-term care and nursing home care
- Seeing patients post-hospital discharge
- Occasional hospice patient care
- Will accept physicians interested in palliative or hospice
- Supervisory role overseeing NPs
- 7 nursing homes: 1 with 80 residents, 1 with 1 resident, others between 40 and 60 residents
- Rounding and resident visits
