- Dates: ASAP-Ongoing
- Schedule: Mon-Thurs (most likely), 0800-1630
- Weekend Requirements: Yes - one weekend per month
- Number of exam room: 4
- Clinic Provider Daily Volume: 16-20
- Average Daily Hospital Practice Census: 0-2
- Appointment length: This can be adjusted to the provider- 15/20 fu and 60 consults
- EMR: EPIC
- Certifications: BC, IA license or IMLC, Fellowship trained
- Clean Malpractice/Background
- Must be able to do MUA
