Patient Population: Medicare, Medicaid, Commercial
Setting: Inpatient & Outpatient Clinic, Telehealth, Outreach
Responsibilities: Patient assessments, new and ongoing treatment plans, therapy adjustments, chemotherapy management, inpatient coverage at both sites, referrals, end-of-life care, pain management coordination
Required Procedures: Medical Hematology/Oncology management, chemotherapy oversight
EMR System: EPIC
Support Staff: 1:1 nursing support; no APPs
Days Per Month: 4–5 days per week; ~6 outreach days/month
On-Call/Weekends: Yes – weekends as needed; call schedule TBD

Submission Requirements:
- Board Certified or Truly Board Eligible in Hematology & Oncology
- Active IA License or IMLC
- Fellowship Trained in Hematology & Oncology
- Clean Malpractice/Background with NPDB self-query
