Practice Setting: Free-Standing Cancer Center

Reason for Coverage: Loss of provider

Dates of Coverage: Starting ~ November 1, 2025 with ideal path toward permanent placement

Call Schedule (during locum period): None

Call Schedule (if converted to permanent): Approximately 1 week of call every 6-8 weeks

Patient Population: Adult, Geriatric

Case Mix: ~65% benign hematology, ~35% malignant oncology, 0% internal medicine

Number of Physicians in Practice: 7

Number of Advanced Practice Providers (APPs): 6

Average Patients Per Day: Outpatient – ~25; Hospital/Rounding/Call – ~20

Clinical Areas: Outpatient oncology, Inpatient rounding/call, Bone marrow transplant oversight, Tumor Board participation, Blood bank, ordering tests & imaging, prescribing, etc.

Documentation System (EMR): EPIC

Credentialing Timeframe: ~60 days
CERTIFICATIONS / LICENSES REQUIRED:

Board Certified in Hematology & Oncology

State license (acceptable if provider will obtain license)
