Practice Setting: Free-Standing Cancer Center

Reason for Coverage: Loss of provider

Dates of Coverage: November 1, 2025 to perm ideally

Call Schedule: No call during locum period; call expected if placed perm; call schedule is 1 week out of every 6–8 weeks

Patient Population for Required Coverage: Adult, Geriatric

Practice Breakdown: 65% benign hematology, 35% malignant oncology, 0% internal medicine

Number of Physicians in Practice: 7

APPs in Practice: 6

Average Patients/Day – Outpatient: 25

Average Patients/Day – Hospital (Call/Rounding): 20

Clinical Area: Oncology – Outpatient & Inpatient

Trauma Level/Designation: N/A

EMR: EPIC
Estimated Credentialing Timeframe: 60 days
Facility Reimbursement for Costs: Flight, Hotel, Rental Car, Mileage (IRS Standard Rate)

Board Certified in Hematology & Oncology – REQUIRED
