- BE/BC Hematologist for Sickle Cell Medical Home program
-One of only a few Sickle Cell Day Hospitals in the country
- Needexperience or interest in caring for adults with Sickle Cell Disease
- Provide acute and chronic disease management for adults with sickle cell disease.
- Must have expertise in managing hematological complications of sickle cell disease (i.e., acute chest syndrome, stroke), expertise in chronic pain management, and expertise in prescribing disease-modifying therapies.
- Participate in the on-call clinical management for sickle cell patients.
-The Sickle Cell Medical Home provides hematology and primary care services, a dedicated sickle cell disease social worker, health wellness coach, and nurse managers to assist sickle cell warriors manage their disease.
-The Sickle Cell Medical Home also addresses acute pain management through the Sickle Cell Day Hospital.
