- Description: Summary: 3 Physician seeking MD to fill the gap and help with access to care and call at least 3-7 days monthly. Bid End Date: 5pm EST Rate Caps for Submission: Malpractice – Must be included in rates. Orientation – half the hourly/callback rate kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” Hourly – 551-600 - “Per hour not OT” Holiday – Must be 1.5x the regular rates “Per holiday hour no OT”Nightly Call – 1001-1200 - “Per night call to hold the pager to include 2.5 hours gratis” Holiday Nightly Call - Per night call to hold the pager to include 2.5 hours gratis” On Call Daily – 3401-3900 - “Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - “Per 24 hours to include 4 hours gratis Call Back – 551-600 - “Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Holiday Call Back – 551-600 “Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Schedule/Availability Requirements:clinic and call5 8-hour days with an average of 2 endoscopy days 3 half endoscopy days and 3 half office dayscall will be shared each month between 3 physicians Facility Location: 1003 Willow Creek Road Prescott Arizona 86301 Setting: Hospital and Office Duties: Evaluate and treat patients in ambulatory clinic settings. Perform Endoscopy procedures during scheduled block time and also while on call. FTE: 0.5 - 1 Required Procedures: ERCP and EUSOffice offers esophageal manometry/video capsule endoscopy/Fibroscan - these are performed by clinical staff and interpreted by physicians EMR: CERNER Support Staff: Medical receptionist Surgery scheduler Medical Assistants Registered Nurse Reason for Coverage: locums taking 6 month break/provider they did backfill with cancelled Deal Breakers: NO ERCP/EUS unable to offer at least one week 7days monthly. Travel Preferences:Preferred Lodging: Preferred hotels Hassayampa HotelAir Travel: Local preferred but not requiredPlease detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission. Credentialing Information: Timeframe: 90-120 days Credentialing with other facilities? No Questions answered by the client: How long does orientation typically take? Standard Hospital/EMR orientation which usually takes 4-6 hours. Do they use Conscious sedation? Or do they use Anesthesia? Both How often will the locum be on night and weekend calls? If this is for the locum to cover calls ONLY the call coverage will consist of 7-15 days monthly. Each call day is 24hrs. Is it 2 full-time needs they are trying to fill? One locum to fill in call coverage gaps only and one full time to provide office and endoscopy services. What does the support staff consist of? Medical receptionists medical assistants and clinic nurses. Endoscopy services covered by hospital/OSC. Cases: EUS/ERCP trained Average Cases/Day: Average of 15 outpatient cases on full Endoscopy days and 8 outpatient cases on half days Office Patients: 8-10 patients 2-3 days/week Inpatients: Fluctuates depending on #of call days Patient Population: Anything from complex IBD and Liver patients to screening for colorectal cancers. A wide range of patients and cases.
