- Clinical Information:
- Inpatient consults/rounding/procedures and outpatient endoscopy (no ambulatory clinic)
- Special Procedures: General GI
- 1-2 days a week/ Outpatient Endoscopy procedures
- IP consults Daily- Rounding, Consults and IP Procedures
- EMR: Epic
- Patient volume: 10-18
- Support staff: MA, LPN, Nurse Practitioner
- Required:
- CV must be in Month / Year format
- Must have license in hand at time of submission, or proof that the license application has been submitted
- Any gaps greater than 30 days will require an explanation on the CV
- DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
