Cost Center: ER On Call – Gastroenterology
Department: Emergency/Trauma

The Coverage will be from 7:00 AM – 7:00 AM.
This will be for general gastroenterology coverage – not need ERCP coverage.

June 28 – 30
July 5 – July 7
Practice Setting: Outpatient: Inpatient: Telehealth: ER
Admissions Required? Yes: No:
Is hospital stroke certified? Yes: No:
Does Provider need to be Board Certified? Yes: No:
Are Temp privileges available? Yes: No:
Do you only want to see CV’s with and Active License? Yes: Active FL License No:
Coverage Type? No Call: Call Only: Scheduled Clinic Hours/Call:
How many Patients will Locum see per day? 6-8 times per week called in on an emergent basis
