Clinic setting: Hospital
18 outpatients/20 inpatient scopes per day
Support staff: 2 physicians, 2 APPs, all services onsite at hospital

Rates to submit:
Hourly rate - "Per hour worked 7a-5p"
On Call Daily rate - "Per 24 hours, to include any clinic hours and night call"
Night call - "Per night to hold pager from 5p-7a"
Weekend call - Saturday & Sunday "Per 24 hours of call, to include 2 gratis hours"
Call back - "Per hour of patient call back hours"
Holidays - 1.5x
Conversion rate
Expenses covered by client will not exceed the limitations listed in the Travel & Expense policy between McLaren Health Care & AHSA.Reverse Invoicing - Provider will use Kronos to clock in/out. No paper timesheets will be used.
Flight, car, fuel, mileage, hotel is a $150 cap (including taxes)Do NOT reimburse Med Staff FeesNo lodging booked through 3rd party websites, ie expedia, kayak, etcMcLaren is tax exempt and will not reimburse taxes on lodging
