Schedule Days Clinic /Hospital 8-5 Monday, Wednesday OR/GI lab
Number of patients seen per day in clinic 15-20
Call Night /Weekend - yes
What are the days for Call Sun-Thurs
What Time does the Call start 7a
What Time does Call end 7a
Call will be for assigned, unassigned, or both assigned and unassigned patients both
Call response time required?( 15 min? 20 min?) 30 min
Call Ratio – Call Back % varies
Average Phone consults while on call varies
Average admissions while on call varies
Average patients seen while on call varies
If there are specific dates you need filled please include them here along with the shift times - for example 6/1-5 8a-5p with night call 5p-8a
Age of patients (Must specify if the provider will see patients under 18) - all to include under 18
