Join a growing medical group as a Nurse Practitioner or Physician Assistant providing primary care for nursing home residents and following patients through skilled recovery stays. Leave the 9-to-5 clinic behind: you set your own day within a 7am to 7pm visit window, with no fixed office hours. Key Responsibilities
- Provide primary care for frail and elderly residents across an assigned group of nursing homes- Lead advance care planning conversations, including POLST
- Coordinate care with families, facility teams, and specialists
Qualifications
- Board-certified NP or PA with a DEA- Geriatric experience, ideally 3+ years
- Adult/Geriatric NP certification, long-term care, or skilled nursing experience a plus
Compensation & Benefits
- $140,000 to $155,000 annually, plus productivity bonus for visits above 260 per month- Health, dental, 401(k), life and disability insurance, and PTO
- $2,000 CME allowance plus paid CME time
- Malpractice, credentialing fees, and mileage paid
- Smartphone, laptop, and hotspot provided
