- Work Dates Needed: ASAP - ongoing
- Preferred Schedule: Mon-Fri (8a-5p)
- Worksite Setting: Outpatient
- Scope of Work:
- General FM practices; annual wellness visits, well child visits, annual CPE visits, chronic disease follow up, acute care visits, transition of care management, in basket cross coverage
- Procedures in-office: joint injections, IUD insertions, implantable contraception, L&D of abscess and skin biopsies
- Prenatal care is optional
- Office located adjacent to urgent care center and OBGYN services are in same location
- 3 FM providers in practice
- Licenses, Certifications, Requirements: Active CT license preferred, IMLC accepted w/LOQ, ABFM, clean files preferred, candidates residing in CT or drive in candidates only
