Hybrid Primary Care Family Nurse Practitioner / Physician Assistant opening in Wethersfield, CT Full-Time | Hybrid Remote/On-Site | Weekdays | No Call A growing primary care team in Connecticut is seeking an experienced Family Nurse Practitioner or Physician Assistant to support hybrid virtual and on-site primary care services. Opportunity Highlights:

• Hybrid Primary Care APP role
• Provide remote Transition of Care visits, Annual Wellness Visits, and same-day primary care visits
• Support traditional primary care services virtually at Campus Care locations
• Hybrid schedule includes remote work from home plus scheduled on-site coverage
• Two days per week on site at assigned Campus Care locations, based on candidate residence and patient needs
• Possible site options include Hartford, West Hartford, New Britain, and New Haven areas
• Nurse or Medical Assistant support available on site or remotely for in-person tasks as needed
• Help expand virtual care services within a growing primary care model
• Full-time weekday schedule
• Occasional rotating weekends
• No call required

• Family Nurse Practitioner or Physician Assistant
• Connecticut license or eligibility
• 5+ years of post-graduate experience as an FNP or PA in a primary care setting required
• Experience with virtual care, transition of care, annual wellness visits, or same-day primary care preferred
• Strong communication, clinical judgment, documentation, and care coordination skills
• Comfortable working in a hybrid remote/on-site care model
