Advanced Practice - Family Medicine position in: Windsor, CT

APP – Family Medicine Primary Care Location: Windsor, CT Position Highlights • Join a collaborative and well-established Family Medicine Primary Care team in Windsor, CT. • Open to Family Nurse Practitioners (FNPs) and Physician Assistants (PAs). • 100% outpatient Family Medicine/Primary Care setting serving patients from birth through geriatrics. • Full-time position with a weekday-only schedule. • No call required. • Supportive practice consisting of 4 physicians and 1 APP. • Embedded behavioral health provider and clinical pharmacist. • Strong collaboration among providers, nurses, medical assistants, and front-office staff. • Team is recognized for an outstanding culture and efficient workflows. APP Role & Responsibilities • Perform routine physical examinations and comprehensive patient assessments. • Develop medical, educational, and psychosocial treatment plans for patients. • Collaborate with physicians and other members of the care team to address patient health needs. • Develop plans for further evaluation, including referrals to specialists and other healthcare professionals when appropriate. • Provide treatment for conditions within the provider’s scope of practice and established protocols. • Identify conditions outside the APP’s scope of practice and appropriately refer to a physician. • Practice under established guidelines and in collaboration with and/or supervision of the practice physicians.
