Primary Care Physician

Sanitas Medical Center is seeking a Board Certified or Board Eligible Primary Care Physician (MD/DO) in Family Medicine or Internal Medicine to join our Windy Hill location in Jacksonville, Florida.

This is an excellent opportunity for a physician who enjoys providing comprehensive outpatient primary care to adult patients age 18 and older. Our physicians work within a collaborative, value-based care environment supported by an integrated healthcare team focused on quality, prevention, care coordination, and improved patient outcomes.

Physician Responsibilities

• Provide comprehensive outpatient primary care to adult patients ages 18 and older

• Diagnose and manage acute and chronic medical conditions

• Provide preventive care, annual physicals, wellness visits, and health screenings

• Manage chronic diseases and coordinate ongoing patient care

• Develop individualized treatment plans based on patient needs

• Coordinate care with specialists and interdisciplinary healthcare teams

• Order and interpret laboratory and diagnostic studies

• Educate patients on preventive health, chronic disease management, and wellness

• Maintain accurate and timely clinical documentation within the electronic medical record

• Collaborate with the care team to support quality initiatives and improve patient outcomes

• Participate in a value-based care model focused on proactive, coordinated, patient-centered care

Qualifications

• MD or DO degree from an accredited medical school

• Board Certified or Board Eligible in Family Medicine or Internal Medicine

• Active Florida medical license or ability to obtain prior to employment

• Active DEA registration or ability to obtain

• Outpatient primary care experience preferred

• Strong communication and patient engagement skills

• Commitment to comprehensive, patient-centered primary care

• Ability to work collaboratively within an integrated care team

• Bilingual English/Spanish preferred, but not required
