Provide comprehensive outpatient care including:

✔ Acute/Sick Visits
✔ Chronic Disease Management
✔ Diabetes & Hypertension Management

✔ Minor Office Procedures
✔ Medication Management
✔ Patient Education
✔ Collaboration With Advanced Practice Providers
