About the Role
Stafingo is seeking Board Certified/Board Eligible Primary Care Physicians (Internal Medicine or Family Practice) to provide on-site care at VA facilities in central Minnesota. We are filling 4 full-time positions: 2 at the St. Cloud VA Medical Center, 1 at the Brainerd Community Based Outpatient Clinic (CBOC), and 1 at the Montevideo Community Based Outpatient Clinic (CBOC). This is a steady, long-term contract engagement (base year plus two option years).
Schedule
Monday–Friday, standard clinic hours 8:00 AM–4:30 PM
Full-time equivalent: 2,080 hours/year per physician
No nights, no call, no weekends
Overtime hours available and paid at a premium rate
Compensation
$175/hour for standard clinic hours
$262.50/hour for overtime hours (1.5x standard rate)

Requirements
Current, full and unrestricted medical license in any U.S. state, territory, or DC (Minnesota license required prior to start)
Board Certification or Board Eligibility in Internal Medicine or Family Practice
Active DEA registration
National Provider Identifier (NPI)
Ability to pass VA credentialing/privileging (VetPro) and background vetting
Current ACLS/BLS certification
Up-to-date immunizations/screenings per CDC guidance
Clean OIG exclusion list status
Fluent English (spoken and written)

Job Responsibilities
CLINICAL DUTIES (approximately 80% of time)
Provide Primary Medicine Care in a team-based, patient-centered model (Patient Aligned Care Team / PACT) — personalized, integrated, comprehensive, and coordinated care, including:
Preventative Care: annual physicals, screenings, and vaccinations.
Acute Care: treatment of common illnesses and minor injuries (colds, flu, rashes, sprains).
Chronic Disease Management: hypertension, high cholesterol, diabetes, and other long-term conditions via regular check-ins, medication management, and test ordering.
Health and Wellness: patient counseling and education on healthy lifestyle, nutrition, and weight management.
Diagnosis, evaluation, treatment, care management, and consultation across settings for: chronic and acute conditions, hypertension, diabetes, hyperlipidemia, joint pain, upper respiratory issues, skin conditions, mental health concerns, other musculoskeletal issues, neurology, gynecology, geriatrics, and ambulatory conditions.
Be physically present on time for all scheduled clinics.
Provide consultation and instruction to referring physicians on appropriate procedure indications; determine treatment course and communicate with referring clinicians.
Initiate social work referrals for homeless veterans and patients without primary care providers.
Follow established medication policies (no sample medications).
Provide discharge education and follow-up instructions coordinated with the next care setting.
Communicate all test results per VHA Directive 1088.
Average workload: approximately 12 patients/day (30–60 minute appointments), plus PACT-related walk-in assessment responsibilities.
ADMINISTRATIVE DUTIES (approximately 20% of time)
Participate in continuous quality improvement activities and committee meetings as required by the Chief of Staff or designee.
Complete QM/PI documentation for all procedures, complications, and outcomes.
Document patient encounters in CPRS, including E&M/procedure coding, within 2 business days of the encounter.
Follow patient safety and infection control standards; report incidents to COR within 24 hours via VA Patient Safety Reporting System.
Refer patient/customer service issues to the Supervisory Physician, Nurse Manager, or designee.
STANDARDS OF CARE
Evidence-based best practices meeting or exceeding standards from the American Board of Internal Medicine, The Joint Commission, American Hospital Association, American College of Physicians, and VHA Handbook 1101.10(2) — PACT Handbook.
PERFORMANCE EXPECTATIONS
100% Board Certification/Eligibility (90% acceptable quality threshold on OPPE standards).
100% scope-of-practice/privileging compliance, reviewed quarterly.
100% on-time/availability for scheduled services, monitored monthly.
Patient safety incidents reported within 24 hours (95% acceptable quality level).
OPPE reviewed quarterly.
Qualifications
LICENSE
Current, full and unrestricted license to practice medicine in any U.S. state, territory, or DC (required onsite on VA property).
Physicians with a license ever restricted, suspended, revoked, voluntarily surrendered pending action, or denied will not be considered.
Contractor must keep the COR apprised of anything that could adversely affect clinical privileges.
BOARD CERTIFICATION
Board Certified/Board Eligible by the American Board of Internal Medicine (ABIM) or American Board of Family Medicine.
Current BLS and/or ACLS certification (American Heart Association) required.
CME documentation must be provided annually to the COR for privileging/re-privileging.
CREDENTIALING AND PRIVILEGING
Credentialed and privileged via VetPro per VHA Handbook 1100.21(1) and VHA Directive 1100.20.
Contractor must furnish an acceptable substitute at no added cost if credentials/privileges lapse, are suspended, or revoked.
TECHNICAL PROFICIENCY
Technically proficient in required clinical skills; fluent in spoken/written English.
Must provide proof of competency, certification, and licensure on request, plus full educational/training history including any gaps.
Must abide by facility Medical Staff By-Laws, rules, and regulations.
MANDATORY TRAINING
Mandatory Training for Transient Clinical Staff (bundle: Active Threat, MST for Medical Providers, Government Ethics, Workplace Harassment/No Fear Act, MRI Safety Level 1, VA Core Values/ICARE, HIPAA Focused Training, Patient Safety, Patient Rights, Patient Abuse, Disruptive Behavior Prevention Level 1, Suicide Risk Management, Bloodborne Pathogens) — annually, 2 hours
VA Privacy and Information Security Awareness and Rules of Behavior — annually, 1 hour
HIPAA — annually
BLS — every 2 years, 4 hours
EHR (CPRS) — one-time, 5 hours
VA Compliance Business and Integrity (CBI) — annually, 1 hour
SCVAHCS Orientation — one-time in-person, 5 hours
HEALTH AND INFECTION CONTROL SCREENING (within 5 days of contract award, current within past year)
Negative TB Skin Test (TST) or IGRA (chest radiograph if positive)
Proof of immunization/immunity: Measles, Mumps, Rubella (or rubella titer ≥1.8)
Proof of immunity: Varicella
1 dose Tdap (Acellular Pertussis)
Annual Influenza vaccine (or mask during flu season if contraindicated)
Bloodborne pathogen training; Hepatitis B vaccination series and surface antigen test results

CONDUCT / ELIGIBILITY SCREENING
Not listed on the HHS OIG List of Excluded Individuals/Entities.
Compliance with Immigration and Nationality Act and E-Verify requirements.
Conduct must not jeopardize patient care; disqualifying issues include intoxication, drug use, theft, patient abuse, or negligence.
SUBMISSION REQUIREMENTS (Technical Factor 1 — per Section E.3)
Proof of successful completion of an accredited residency in Internal Medicine or Family Practice.
CV demonstrating required clinical experience and competency in the PWS scope of care.
Malpractice history disclosure.
