Start Date: As soon as credentialed
Contract Length: 6 mo, option to extend
Shift Details: 5x8s
Schedule: M-F 8a-4p slight flexibility (7a-3p or 9a-5p) - will not consider 10hr shifts at this time
Call requirements: No Call
Work Details:
• Support staff: MAs, LVNs, all providers have an MA, MA has support of LVN backing them as needed.
• Amount of coverage needed per month: Full time coverage
• Patients per day: 23-25 pts/day or 3 pts/hr w/ 30 min admin time
• EMR: Epic
Practice Setting:
• Clinic
Requirements:
• Board Certification: BC Required
• License required: CA license; open to licensing
• DEA: CA DEA required
• Minimum experience required: Will consider new grads who are board certified (will not accept BE)
Credentialing Timeframe: 30-45 days
