Position Summary
A Family Medicine Physician is sought to provide comprehensive, long-term care to a diverse patient population. This role focuses on diagnosing and managing both acute and chronic conditions, while building strong, ongoing relationships with patients in a patient-centered, value-based care environment.

Key Responsibilities
* Evaluate patients to assess symptoms and health concerns
* Diagnose and treat acute illnesses and chronic diseases (e.g., hypertension, diabetes, heart disease)
* Prescribe medications, therapies, and preventive treatments
* Provide immunizations and preventive care services
* Educate patients on health, wellness, and disease prevention
* Monitor patient progress and adjust treatment plans as needed
* Maintain accurate and detailed medical documentation
* Coordinate care with specialists and other providers
* Support patients receiving long-term or complex care

Qualifications
* MD or DO degree from an accredited medical school
* Eligible for or active Utah medical license
* Strong diagnostic and clinical decision-making skills
* Excellent communication and interpersonal skills
