Seeking BC/BE Family Medicine or Internal Medicine Physician to join post-acute care team in Port Charlotte, Florida
Position Highlights:
Full-time schedule, 5 days per week
Skilled nursing facility setting
Focus on post-acute patient care and care coordination
Collaborative team environment with physicians, nursing staff, and interdisciplinary providers
In-person role with local patient population
Responsibilities:
Perform comprehensive patient assessments including history and physical exams
Develop and manage individualized care plans
Monitor patient progress and adjust treatment as needed
Coordinate care with interdisciplinary team
Educate patients and families on health management and preventive care
Maintain accurate and timely medical documentation
Qualifications:
Board Eligible or Board Certified in Family Medicine or Internal Medicine
Active Florida license and DEA or ability to obtain
Experience in post-acute, acute, or primary care preferred (new graduates welcome)
Compensation & Benefits:
Salary range $235,212 to $278,254 annually
