- Other details: The facility is looking for a provider to cover on-site clinical hours only. There are an average of 16 patients per shift. Candidate CVs must include the month and date for all work experience, with any gaps in employment accounted for. Responsibilities include some chronic pain management chart closure within 72 hours. The schedule is Monday through Friday, 8:00 am to 5:00 pm, with a minimum of 18 patients per day, including a mix of pediatric and adult patients (family medicine only will be considered). Candidates may be MD or DO, Board-Eligible within 5 years of residency will be considered. An active DEA is required prior to the start date. Required immunizations for credentialing include flu vaccine, TB status/X-ray results within 90 days, positive MMR titer or complete immunization series, positive Hep B titer or complete immunization series, positive Varicella Zoster titer or complete immunization series, and Tdap immunization within the last 10 years.
