Lead Physician - Population Health / Value Based Care

A state of the art, Primary Care Medical Center/Patient Centered Medical Home located in Cincinnati, OH, is seeking a Lead Primary Care Physician to join their growing team. This role will be ~50% clinical/~50% administrative.

- In-house Pharmacy
- IV and Infusion treatment
- Diagnostic testing
- Transportation door to door
- Psychiatrist services for Behavioral/Mental Health and Substance Abuse patients
- Psychologist on staff
Schedule:
- Monday – Friday, 8am – 5 pm, 1 hour for lunch
- NO weekends, evenings or holidays required!

Lead Physician Responsibilities:
- Collaborate with an interdisciplinary team consisting of nurse practitioners, psychiatrists, social workers, RN case managers, medical assistants, administrative staff, etc.
- Supervise and coach a team of Nurse Practitioners as they provide primary care services for an underserved/at-risk population
- Provide primary care services to a panel of members
- Oversee clinical staff development and performance
- Help reduce preventable ER visits and hospitalizations
Qualifications:
- An active medical license in the state of Ohio
- Board certified in Internal Medicine or Family Practice
- 5+ years of experience as a Primary Care Physician
- Desire to work with the underserved
