Internal Medicine/Family Medicine/Geriatrics Physician - Post-Acute Care – Wilmington, DE | Flexible Schedule

Opportunity Highlights:
• Full-time (5 days/week) post-acute care role across SNF and ALF settings
• Provide primary care rounding in 2 skilled nursing facilities and 1 assisted living facility
• Flexible rounding schedule with autonomy to structure your day
• Manage chronic and acute conditions while developing care plans
• Collaborate with interdisciplinary teams including clinicians and facility leadership
• Build relationships with patients, families, and care teams to improve outcomes
• Provide mentorship and oversight to nurse practitioners
• Participate in quality improvement and care coordination initiatives

Compensation & Benefits:
• Estimated annual compensation: $210,579 – $271,579+ with uncapped earnings

Qualifications:
• Board Eligible/Certified in Internal Medicine, Family Medicine, or related field
• Active Delaware license (or ability to obtain)
• Active DEA (or ability to obtain)
• Post-acute or similar experience preferred, but not required
