Job Responsibilities
• Manage chronic conditions, including chronic pain management, and complete chart closure within 72 hours.
• Work Monday–Friday, 8:00 a.m. to 5:00 p.m. with no call requirements.
• Collaborate with an on‑site team consisting of 1 registered nurse and 2 medical assistants.
• Rotate between the Fortuna Community Health Center and Ferndale Community Health Center as needed.

• Board certified or truly board eligible in Family Medicine or Internal Medicine (within 5 years of residency); DO candidates will be considered.
• Active DEA license and Basic Life Support (BLS) certification.
• Ability to complete credentialing within 30–90 days (60+ days typical).
• Required immunizations: current flu vaccine; recent TB status or chest X‑ray (within 90 days); positive MMR, Hepatitis B, and Varicella Zoster titers or completion of immunization series; and Tdap within the last 10 years.

• Comfortable seeing both pediatric and adult patients and managing chronic pain.
