Seeking a physician to join a post-acute care team in the Rochester, New York area. This position focuses on rounding in skilled nursing facilities while working within a supportive clinical environment that offers autonomy, flexibility, and strong earning potential.
Position Highlights:
Full-time position (5 days per week)
Rounding in skilled nursing facilities

Responsibilities:
Perform comprehensive patient assessments including medical history and physical exams
Develop and manage individualized care plans
Monitor patient progress and adjust treatment plans as needed
Collaborate with nursing staff and interdisciplinary healthcare teams
Maintain accurate documentation and medical records
Qualifications:
MD or DO
Eligible for New York medical licensure
Local to Rochester, NY or willingness to relocate
Strong clinical, communication, and teamwork skills
Compensation & Benefits:
Competitive base salary: $243,488 – $288,186 annually
