- Shift Schedule: 2 days per week; PRN basis - Half day (4 hrs) or - Full day (4-8 hrs)
- Duties: - Perform one-time non-treatment C&P exams & separation health assessments. - Exams are primarily interview-based with some physical assessment: range of motion testing, muscle strength testing. - No prescribing, no treatment
- Other Info: - Willing to accept: FM/IM/Orthopedic Surg/Anyone w/ General Med OR compensation & pension experience.
- Malpractice Insurance Covered
Requirements:
- Active KY License
- BC/BE
- Own a laptop (No MACs/Apple products)
- DMA Certification or willing to obtain
- Provider must have their own office
- Proficient in METs testing, evaluating & diagnosing respiratory, cardiovascular.
- Must be within 50 miles from the facility
- CV w/ current home address listed
- Copy of ECFMG cert (if attended school in a diff country)
- Self-Query NPDB (pulled within 30 days)
