Location: Upland, CA
Schedule: Monday–Friday, 8am–5pm
Hours: 40/week
Contract: 3 months (high likelihood of extension or locums-to-perm)
Position Overview
Seeking an experienced provider for a geriatric primary care setting. Must have strong HEDIS knowledge and a solid understanding of the Value-Based Care (VBC) model. Excellent opportunity for long-term extension or transition to perm for the right fit.
