Join a mission-driven, non-profit Federally Qualified Health Center (FQHC) dedicated to providing compassionate, quality care to North Central Massachusetts. We are seeking a full-time Family Medicine Physician to provide comprehensive primary care to a diverse patient panel. This is an opportunity to practice in a collaborative, multi-site environment that integrates behavioral health, dental, and specialty services.

-Schedule: Full-time; primary care focused with the potential to pick up Urgent Care shifts.
-Locations: Choice of five sites across Fitchburg, Gardner, and Leominster, MA.
-Team: Collaborative model working alongside Nurse Practitioners, Clinical Psychologists, and a full support staff.
-Mission: Providing care regardless of income or insurance status across 36 communities.


  • Sign-On Bonus: $30,000 (For Full-Time; eligible if not through a recruiter).

  • Relocation Assistance: Up to $5,000 reimbursement.

  • Loan Repayment: Up to $25,000 annually for the first 5 years (subject to eligibility based on other federal/state forgiveness programs).

  • Professional Allowance: $3,500 annually for CME/Professional expenses.

  • Reimbursements: 100% coverage for MD License, DEA, and MCSR renewals.

Job Responsibilities

-Provide comprehensive primary care, including examinations, assessments, diagnoses, and treatment.
-Manage a continuous panel of patients within a team-based, collaborative care model.
-Prescribe medications and treatment regimens; manage drug dosages and patient education.
-Coordinate patient care activities with nursing and clinical support staff.
-Refer patients to internal specialists (Optometry, Podiatry, Behavioral Health) and external components as needed.
-Participate in the shared On-Call coverage rotation.
-Advise and train subordinate clinical professionals or students in your area of expertise.
-Participate in health promotion, disease prevention, and quality improvement programs.

Qualifications

-Education: Medical Degree (MD or DO).
-Board Certification: Board Certified or Board Eligible in Family Medicine.
-Licensure: Current licensure by the Massachusetts Board of Registration in Medicine.
-Certifications: ACLS or CPR/AED certified; Current DEA, DEA-X, and Massachusetts Controlled Substance (MCSR) licensure.
-Credentialing: Eligible for unrestricted health center and hospital privileges at affiliated facilities.
-Communication: Effective verbal and written communication skills for a diverse clinical team

Working Hours

-Shifts: Standard clinical hours with flexibility for Urgent Care opportunities.
-On-Call: Participation in the group after-hours rotation.
-Placement: Full-time permanent position

-Proficiency in full-spectrum family practice and chronic disease management.
-Strong clinical leadership and the ability to mentor clinical students/subordinates.
