Post Acute Care Opening in Middletown, DE Wilmington 24m We are seeking a compassionate and driven physician to join our post-acute care team in the Middletown, Delaware, area. This is a full-time opportunity (5 days/week) rounding in skilled nursing facilities, with excellent support and training, autonomy, and earning potential. * Full-time (3.5-5 days per week) * Day call and weekend rotation * Flexible rounding schedule * Expected compensation is estimated salary range of $210,529 - $270,529 annually with no cap on productivity income potential. * Uncapped earning potential * Excellent benefit package * Paid professional liability insurance with tail coverage Duties * Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care * Collaborate with clinicians, facility leaders and others regarding care and services * Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery * Provide oversight, guidance, and mentorship to our nurse practitioners * Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care * Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues Qualifications * Current physician license (state of Delaware) and DEA * Experience in post-acute, acute, ED, or clinic settings preferred but not required; new grads welcome to apply * Board certified * Strong clinical foundation and knowledge of healthcare regulatory standards * Excellent communication and interpersonal skills * Collaborative, team-oriented approach with a positive outlook Middletown, DE Located 24 miles south of Wilmington, Middletown is a small town in New Castle County, Delaware, with a population of about 20,000.
