Job description Primary Care Physician – Post-Acute Care Opportunity Location: Syracuse, New York Position Type: Full-Time, W2 Work Setting: In-person Position Overview Seeking a Primary Care Physician to provide care in a post-acute care setting in Syracuse, New York. Schedule & Work Environment
  • Full-time physician role

Clinical Responsibilities
  • Perform patient assessments, medical histories, and physical exams
  • Develop and adjust individualized care plans
  • Monitor patient progress and coordinate care for optimal outcomes
  • Educate patients and families on health management
  • Maintain accurate and timely medical records
  • Ensure compliance with state regulations and facility standards
