Job Responsibilities
• Diagnose, treat, and manage acute and chronic conditions
• Coordinate specialty referrals, hospital transitions, SNF, DME, and home health
• Participate in daily team huddles and quality improvement initiatives
• Maintain accurate, timely EMR documentation
• Review outcome metrics and quality performance data
• Deliver patient-centered care in a value-based model
Qualifications
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active and unrestricted medical license (or eligible)
• Active DEA license
• Medicare & Medicaid provider eligibility
• Strong interpersonal and communication skills
• Commitment to team-based, value-driven care
