Compensation & Benefits

• Associate Medical Director: $273,488 – $318,185 (uncapped productivity)
• Medical Director: $310,000 – $345,000 (uncapped, fee-for-service model)
• Sign-on bonus available

Responsibilities

• Provide post-acute rounding in skilled nursing facilities
• Support and oversee advanced practice providers
• Lead or participate in clinical case reviews and care coordination
• Partner with facility leadership on quality and outcomes initiatives

Qualifications

• Board Certified in Family Medicine, Internal Medicine, Geriatrics, or related field
• Active New York license and DEA (or ability to obtain)
• Leadership experience preferred for Medical Director role
• Post-acute, hospital medicine, hospice, or home health experience a plus
